Adult New Patient Questionnaire

Child (under 16 years old) New Patient Questionnaire

 

·         If you wish to register at our practice, please either attend the surgery to collect our registration pack, or download from above the Registration form and the New Patient Questionnaire above if you are over 16 years of age and the Child Questionnaire if you ar eunder 16 years of age, this needs to include your child's immunisations and any regular medication they take. We will also need to check that you live in our catchment area, so please send a proof of address if you have it. 

·         If you are unable to download the required forms, please visit either Gervis Road or St Leonards Road surgery where you will be able to collect the forms required.

 

·         If you have your own blood pressure machine or have taken it recently, please send in your BP reading if possible, for your records.

·         Once registered with the practice you will be assigned a named accountable GP.

·         You will still have the right to express a preference to see a particular GP of your choice, however please note that some services are not provided by all doctors and in some cases, due to demand for a particular GP, you may not always be able to see your preferred GP.

·         For example, please remember we operate two surgeries with the same doctors and that not all doctors work full time.

·         It is practice policy that where a child is registered at the practice at least one parent is present at the child’s registration and also registers with the practice.
