Nurse Practitioners
Our nurse practitioners are independent practitioners/prescribers that can offer treatment or advice just as a GP would. They are a registered nurse that has received additional university training, so they can diagnose, assess and treat patients with acute problems.

Community Nurses
The community nurses are based at Kings Park Hospital. They provide nursing care and treatments for housebound patients with complex, long-term conditions in their own homes. Requests for attendance can be made through the surgery. There are some services that the District Nurses do not provide, however ‘Help & Care’ is a voluntary service which gives patients access to a wide variety of services. They can be contacted on 0300 111 3303.
Health Visitors
Health visitors provide information, advice and support to pregnant women, expectant fathers and parents of new babies and young children. Health visitors are specialists in nursing and/or midwifery. They work with parents and families to ensure the physical and emotional wellbeing of children up to the age of five, covering antenatal support, parenting support, health checks and development reviews.
The Assessment and Care Team
The Assessment and Care Team, (AC Team) is a small team of nurses that visit you at home. Your GP will refer you to them if they think that you may benefit from a visit from us. Initially, we arrange an appointment to do a full assessment of your health and home circumstances to ensure that you are receiving the best possible care that is tailored to your individual needs. We work with your GP to arrange any referrals or tests that you may need, and we can advise you and your family of the range of local council and voluntary services that is available. We will then visit intermittently, depending on your needs, to ensure you continue to receive the best support available for you.
